Xylitol (birch sugar): toxic to dogs

Xylitol is highly toxic to dogs: even small amounts cause hypoglycaemia and liver failure. What the science says and what to do if ingested.

What is xylitol?

Xylitol is a sugar alcohol that occurs naturally in small amounts in fruit and tree bark, but is commercially extracted from birch wood — hence the name 'birch sugar'. For humans, it has benefits: fewer calories than sugar and an inhibitory effect on dental caries. For dogs, however, it is a serious poison.

Products that may contain xylitol:

  • Sugar-free chewing gum and sweets
  • Toothpaste and mouthwash (including some pet toothpastes — always check the label)
  • Certain peanut butter brands and nut butters
  • Baked goods, energy bars and biscuits with a 'sugar-free' indication
  • Vitamin and supplement preparations (including chewable tablets)
  • Nasal sprays and certain medicinal products

Why is xylitol dangerous for dogs?

In humans, xylitol has hardly any effect on blood glucose levels. In dogs, it works fundamentally differently: xylitol stimulates the pancreas to release insulin rapidly and powerfully — even without an actual rise in blood glucose levels. The result is dangerous, rapidly occurring hypoglycaemia (low blood sugar) that can be fatal without treatment [1].

At higher doses — and sometimes even at lower doses — xylitol also causes direct liver damage, presumably due to oxidative stress in liver cells. A JAVMA case series described eight dogs that developed acute liver failure and coagulation disorders after xylitol ingestion; in some animals the outcome was fatal [2].

There is no known safe lower limit. Any suspected xylitol ingestion in a dog must be treated as potentially emergency [1].

Which animal species are at risk?

Dogs are the most documented risk group. Whether cats and other mammals have the same sensitivity is insufficiently researched; caution is advised with all pets [1].

Symptoms of poisoning

Hypoglycaemia can occur as early as within 15 to 30 minutes after ingestion. Liver damage often does not manifest until 12 to 24 hours later [1][2].

What to do in case of suspected ingestion?

Contact a veterinary surgeon immediately — even if the dog is not yet showing symptoms. The earlier treatment starts, the greater the chance of recovery.

Try to determine:

  • Which product and how much the dog has ingested
  • How long ago (as accurately as possible)
  • The weight of the dog

Do not give the dog anything to eat or drink and do not attempt to induce vomiting yourself without consulting a veterinary surgeon — this can cause additional damage in certain situations.

'Natural' does not mean safe

Xylitol occurs in nature and has proven benefits for humans. However, the indication 'birch sugar', 'plant-based' or 'natural sweetener' on a label offers dogs no protection whatsoever. Always check ingredient lists of products that come into the vicinity of dogs [1].

Why this verdict?

Grade A is justified based on an authoritative narrative review from 2018 summarising pharmacokinetics and clinical mechanisms [1], supplemented by a JAVMA case series of eight dogs with acute liver failure and coagulopathy following xylitol ingestion [2]. Controlled RCTs are ethically unfeasible for this poisoning question; nevertheless, the consistency of findings across multiple case series and the well-substantiated mechanism (insulin dumping independent of blood glucose) justify the highest evidence grade.

Interactions with medication

  • Xylitol enhances the hypoglycaemic effect of insulin and oral hypoglycaemic agents — dogs using such medication are at extra risk upon ingestion.
  • Products containing both xylitol and other sugar alcohols (sorbitol, maltitol) may delay gastric emptying, which can affect the time to symptoms but does not remove the danger.

Legal status (Netherlands)
