What is nutritional therapy?

Nutritional therapy — also therapeutic diet treatment — uses the composition of food as a clinical instrument in the treatment or support of disease. It goes beyond general 'healthy eating': therapeutic diets are formulated for a specific condition, with adjustments in protein content, fatty acid profile, fibre, minerals or fermentable substrates.

In dogs and cats, nutritional therapy is most commonly used for:

  • Chronic kidney disease (CKD) — renal diets with restricted phosphorus and adjusted protein quality
  • Osteoarthritis — omega-3-enriched diets or fish oil supplements for pain relief
  • Food-responsive diarrhoea and food hypersensitivity — hydrolysate or novel protein diets
  • Inflammatory bowel disease (IBD) and diabetes mellitus — diet modulation as an adjunct to medication

What does the evidence say?

Chronic kidney disease in cats

The strongest evidence in veterinary nutrition comes from a double-blind RCT in cats with CKD [1]. The renal diet resulted in a 44% reduction in total mortality compared to a standard maintenance diet; no uraemic crises were reported in the diet group. This is one of the few veterinary nutrition studies with a design comparable to human medicine.

Osteoarthritis: omega-3 works, glucosamine does not

A meta-analysis of 72 trials (57 articles) confirmed clear analgesic efficacy of omega-3-enriched diets and supplements in osteoarthritis in dogs and cats [2]. Notably: the same analysis concludes that glucosamine-chondroitin shows no clinically relevant analgesic effect in dogs or cats and is no longer recommended for pain management. This is an important difference from popular expectation.

Gut health and microbiome

Diet composition demonstrably affects the gut microbiota in dogs and cats [3]. In food-responsive diarrhoea, hydrolysate and novel protein diets are first choice, but correct selection requires veterinary diagnosis — 'grain-free' or 'hypoallergenic' on the packaging is no guarantee of therapeutic suitability.

Limitations and risks

Nutritional therapy is adjunct to, not a replacement for medical treatment in chronic conditions. Independently instituting therapeutic diets — without diagnosis and monitoring — increases the risk of:

  • Nutrient imbalances in home-prepared, raw or vegan diets; deficiencies in taurine, essential amino acids, vitamins and minerals have been described
  • Undesirable protein restriction in already undernourished cats with CKD
  • Electrolyte disturbances with incorrect use of renal diets

Practical

Therapeutic diets are prescribed by the veterinary surgeon based on diagnosis and blood values. Dietary adjustment without an underlying diagnosis or in an animal that is already ill can cause harm. Monitoring of weight, kidney function and electrolytes is part of any long-term therapeutic diet.

Why this verdict?

Evidence grade A is based on a double-blind RCT in cats with chronic kidney disease [1] and a systematic review with meta-analysis of 72 trials on osteoarthritis in dogs and cats [2]. The available studies concern specific conditions; for other dietary interventions the evidence is less strong or limited to observational and narrative research [3].
