Why is vitamin D dangerous?

Vitamin D3 (cholecalciferol) is essential for bone and calcium metabolism in the correct amount. However, at too high doses it stimulates excessive calcium absorption from the intestine and mobilisation of calcium from bone tissue. The result is hypercalcaemia: a too high calcium level in the blood. That calcium subsequently deposits in soft tissues — kidneys, heart and blood vessels — and causes progressive organ failure [4].

The danger of vitamin D is that it is fat-soluble and accumulates in the body. One large dose or prolonged exposure to slightly elevated levels both lead to poisoning.

Three poisoning routes

1. Rodenticides based on cholecalciferol

Cholecalciferol rodenticides are legally available and extremely potent for dogs and cats. Even a small amount is dangerous [4].

2. Contaminated commercial feed

In 2019, Hill's Pet Nutrition recalled almost 22 million cans of dog food. Laboratory analysis showed vitamin D3 levels of more than 33 times the safe upper limit — the result of an error with a vitamin premix from a supplier [3]. A similar incident affected five kittens eating kibble with a vitamin D3 content 44 times above the FEDIAF maximum limit; two of them were left with permanent chronic kidney insufficiency [1].

3. Overdosing of supplements

Supplementation without veterinary guidance is a real risk. Because vitamin D3 accumulates, 'a little bit extra' can still lead to poisoning over weeks.

With chronic exposure via contaminated feed, symptoms can be more subtle: gradual weight loss, dullness, increasing thirst [1][2].

Treatment

Early treatment determines the outcome. With recent ingestion, a veterinary surgeon can induce vomiting and administer activated charcoal to limit further absorption. Subsequent treatment consists of intravenous fluids, furosemide (to excrete calcium via the urine), prednisone and a low-calcium diet. In severe cases, bisphosphonates are used, which can lower serum calcium levels within 24–48 hours [2][4]. If kidney failure is already present, the prognosis is unfavourable.

'Natural' does not mean safe

Vitamin D3 is sometimes promoted as a 'natural supplement' for pets. That is true insofar as it is an endogenous substance — but the margin between an effective dose and a toxic dose is much smaller in dogs and cats than in humans. Never supplement without explicit dosage advice from a veterinary surgeon, and when buying commercial feed check whether there are any active recall actions via the FDA or NVWA.

What to do if poisoning is suspected

Call a veterinary surgeon or the nearest veterinary emergency number immediately. Take the packaging of the feed or supplement with you. Do not wait for symptoms to worsen — the earlier treatment starts, the better the prognosis [4].

Why this verdict?

Evidence grade A is based on multiple published case series with clinically confirmed diagnoses [1][2], a large-scale FDA recall with laboratory measurements of toxic levels in commercial feed [3], and extensive veterinary toxicological literature [4]. Limitations: controlled dose-response studies in companion animals are absent for ethical reasons, but the clinical and analytical data are consistent and mechanistically well substantiated.

Interactions with medication

  • Concurrent use of other calcium or vitamin D supplements cumulatively increases the risk of poisoning
  • Diuretics such as furosemide are used therapeutically but must not be administered independently
  • Corticosteroids (prednisone) inhibit calcium absorption and are used only under veterinary supervision

Legal status (Netherlands)

Under Dutch law, cholecalciferol rodenticides are legally available for consumer use; keep them out of reach of pets. Commercial animal feed is subject to FEDIAF maximum standards for vitamin D; if contamination is suspected, a complaint can be filed with the NVWA (Netherlands Food and Consumer Product Safety Authority).
