What is TCVM?
Traditional Chinese Veterinary Medicine (TCVM) is an umbrella term for diagnostic and therapeutic methods derived from traditional Chinese medicine, applied to animals. The main components are:
- Acupuncture and acupressure — stimulation of specific body points with needles, heat (moxibustion) or pressure
- Chinese herbal medicine — preparations of plant, animal and mineral origin
- Tui na — Chinese therapeutic massage
- Dietary therapy — food selection based on 'thermal properties' and the theory of the Five Elements
- Qi gong — breathing and movement therapy (rarely applied in animals)
TCVM is based on concepts such as Qi (life energy), Yin/Yang and the Five Elements. These concepts have no demonstrable biological equivalent in contemporary physiology or biochemistry.
For which complaints is TCVM used?
Practitioners typically apply TCVM for:

What does the evidence say? (Grade: Insufficient)
The level of evidence for TCVM as a whole is Insufficient. This means: there are too few well-designed studies to reliably determine the effect — positive, neutral or negative.
Specific shortcomings in existing research:
- Study quality — most veterinary studies are small, non-blinded and lack an adequate control group; as a result, effects are systematically overestimated
- Placebo and observer bias — owners know which treatment their animal receives, which demonstrably colours their assessment of recovery
- Species specificity — findings from human TCM research are not simply transferable; cats, dogs, horses and rodents differ fundamentally in anatomy and metabolism
- Publication bias — positive results are published more often than negative ones, which distorts the available picture
Acupuncture has received the most — but still limited — scientific attention. For Chinese herbal medicine in animals, the evidence base is even narrower.
Insufficient evidence is not evidence of no effect. It is an honest statement that science currently provides no reliable answer.
Safety
The safety profile varies greatly per component.
Acupuncture
- Relatively low risk when performed by a trained veterinary surgeon
- Possible risks: infection at needle site, broken needles, stress or pain in the animal during treatment
Chinese herbal medicine
- Increased risk due to quality variation — preparations may contain undeclared substances, heavy metals or residues of conventional medicines
- Cats are particularly vulnerable: due to limited hepatic glucuronidation, they metabolise many plant compounds poorly or not at all, which rapidly leads to toxicity
- Interactions with conventional medication (see below) are possible and clinically relevant
- Always inform the veterinary surgeon about all herbs and supplements the animal receives
General
Never delay conventional diagnostics or treatment in favour of TCVM in cases of acute or unexplained deterioration.
Legal framework
In the Netherlands, making a diagnosis and treating an animal falls under the Animals Act (Wet dieren, 2011) and is reserved to registered veterinary surgeons. TCVM procedures — including acupuncture and the prescribing of herbal preparations for therapeutic use — fall under this legal definition. Therapists without BIG (Big Register of Healthcare Professions) or KNMvD (Royal Dutch Society for Veterinary Medicine) registration operate in a legal grey area when applying therapies. Owners bear responsibility themselves for the choice of practitioner.
Why this verdict?
The available literature on TCVM in animals consists almost exclusively of small, unblinded studies with weak control groups — study types that systematically overestimate the chance of a positive result. Systematic reviews on related CAM modalities repeatedly show that methodological shortcomings do not allow definitive conclusions. For Chinese herbal medicine in animals, the evidence base is even narrower than for acupuncture, and extrapolation of human TCM data is not valid due to fundamental anatomical and physiological differences per animal species.
Interactions with medication
- Chinese herbal preparations can interact with anticoagulants (e.g. warfarin)
- Possible interactions with NSAIDs and corticosteroids when used concurrently
- Medicines metabolised via cytochrome P450 can be affected by certain herbal extracts
- Cats: increased risk of toxicity due to reduced metabolism of plant compounds
Legal status (Netherlands)
Veterinary treatment — including acupuncture and therapeutic use of herbal preparations — is reserved in the Netherlands to registered veterinary surgeons (Wet Dieren 2011). Therapists without veterinary authority may not make a diagnosis or perform treatment.
