What are SAMe and silybin?
SAMe (S-adenosylmethionine) is a substance that occurs naturally in the body and plays a key role in methylation reactions and the production of glutathione — an important antioxidant in liver tissue. In liver diseases, endogenous SAMe production decreases, making liver cells more vulnerable to oxidative damage [4].
Silybin is the most biologically active component of silymarin, the active flavonolignan complex from milk thistle (Silybum marianum). Silymarin exerts antioxidant and anti-inflammatory effects and inhibits the uptake of toxins into liver cells [1][2].
The combination SAMe + silymarin is commercially available in veterinary registered supplements. Both substances are also available separately.
Mechanism of action
- SAMe restores intracellular glutathione levels and supports methylation processes that are essential for liver cell function [4].
- Silybin protects liver cell membranes against toxins and free radicals, and has antifibrotic properties [1][4].
Both substances complement each other: SAMe acts primarily via intracellular restoration, silybin via membrane protection and direct antioxidant activity.
Evidence per species
Dog
The strongest evidence comes from a randomised clinical trial (RCT) in dogs undergoing chemotherapy (CCNU): the combination SAMe/silymarin reduced the chance of significant liver enzyme elevations compared with placebo [3]. Narrative reviews support the biological plausibility and describe positive clinical experiences in chronic liver diseases and toxin exposure [1][2][4]. However, the total amount of RCT evidence remains small and the one available study is specifically focused on chemotherapy patients — generalisation to other liver diseases requires caution.
Cat
For cats, direct evidence is more limited. Reviews describe use in toxic liver diseases and chronic liver disease [2][4], but large-scale randomised studies in cats are lacking. The physiological rationale is comparable to that in dogs — liver cell damage also leads to glutathione deficiency in cats — but extrapolation remains cautious.
Safety
Both substances are generally considered well tolerated in dogs and cats when used as recommended [1][2][4]:

Important: Cats metabolise many substances differently than dogs (including limited glucuronidation). Use only products that are specifically formulated for cats or recommended by a veterinary surgeon. Always consult a veterinary surgeon regarding the correct dosage — this is species- and weight-dependent.
When is this considered?
SAMe and silybin are considered in (complementary) veterinary medicine for:
- Chronic liver diseases (hepatitis, liver fibrosis)
- Exposure to liver toxins (e.g. xylitol, certain medicines)
- Support during long-term use of liver-loading medication
- Elevated liver enzymes (ALT, ALP) after exclusion of other treatable causes
This is explicitly not a replacement for veterinary diagnostics or treatment. An elevated liver enzyme always first requires a diagnosis by a veterinary surgeon before supplements are considered [4].
Why this verdict?
Grade B is supported by one randomised clinical trial in dogs receiving chemotherapy [3] and several narrative reviews [1][2][4] describing the mechanism of action and clinical use. However, the body of evidence is limited in size and population diversity: the only RCT focuses on a specific group (CCNU chemotherapy patients), and for cats randomised research is almost entirely absent. Consequently, the evidence is promising but not robust enough for grade A.
Interactions with medication
- SAMe and serotonergic drugs (tramadol, certain antidepressants): increased risk of serotonin syndrome [2]
- Silymarin may affect the breakdown of drugs via CYP450 enzymes; relevant in polypharmacy [1][4]
Legal status (Netherlands)
In the Netherlands, SAMe and silymarin are available as dietetic feed or dietary supplements for companion animals. They do not fall under the Wet dieren (Animals Act) as medicinal products. Treatment advice and dosing are legally reserved to the veterinary surgeon (art. 4.1 Wet dieren).
