What is melatonin?

Melatonin is a hormone produced in mammals by the pineal gland. It regulates the day-night rhythm and plays a role in seasonal physiological processes, including coat growth and reproduction. As a supplement, melatonin is available in tablets, capsules and implants.

What is it used for in animals?

In dogs, the three most commonly cited applications are:

  • Alopecia X (also known as "black skin disease" or hair cycle arrest): a form of non-inflammatory hair loss in fluffy breeds such as the Pomeranian, Keeshond and Samoyed
  • Seasonal flank alopecia: periodically recurring hair loss on the flanks, presumably light-sensitive
  • Nocturnal restlessness in cognitive dysfunction (canine dementia): as a sleep-supporting agent

In cats, melatonin is sometimes anecdotally recommended for behavioural regulation or suppression of the oestrous cycle, but published evidence in cats is virtually absent.

What does the research say?

Alopecia X in dogs

An open case series (n=29 dogs, predominantly Pomeranians) reported partial to complete hair regrowth in 62% of participants after oral melatonin administration [1]. Hormone levels did not necessarily normalise. A more recent narrative review confirms that there are indications of an effect, but points to methodological limitations of the available studies [2]. Without a randomised control group, it is unclear to what extent spontaneous recovery — which occurs regularly in Alopecia X — distorts the outcomes.

Seasonal flank alopecia

A double-blind, randomised, placebo-controlled study (n=17 dogs, Netherlands, 2022) investigated whether a subcutaneous melatonin implant could prevent recurrence of seasonal flank alopecia. No statistically significant difference was found between the melatonin and placebo groups [3]. This is the methodologically strongest study in this field, and the result gives reason for caution.

Nocturnal restlessness and behaviour

For use in nocturnal restlessness in the context of cognitive dysfunction in dogs, melatonin is used in practice, but direct controlled trials are lacking. The rationale is based on human sleep medicine and the biological plausibility of melatonin as a circadian regulator.

Safety and points of attention

Melatonin itself has a wide safety margin in healthy dogs and cats. The greatest practical risk factor is the product itself, not the active substance: human melatonin supplements regularly contain xylitol or other sweeteners that are seriously or fatally toxic to dogs. Use only animal-safe products, preferably after consultation with a veterinary surgeon.

Hormonal interactions are possible in animals with disorders of the thyroid gland, adrenal cortex or reproductive system. In these animals, use should only occur under veterinary supervision.

When to see a veterinary surgeon?

Hair loss in dogs and cats can have dozens of causes, including infections, parasitic conditions, thyroid abnormalities and immune diseases. Using melatonin independently without a diagnosis is not recommended. A veterinary surgeon can assess whether Alopecia X or seasonal flank alopecia is plausible and whether melatonin is a sensible option as part of a treatment plan.

Why this verdict?

The evidence is assessed as grade C (weak). Available studies are small in size (n=17–29), partly without a control group, and the only double-blind RCT for seasonal flank alopecia showed no significant benefit of melatonin over placebo [3]. For behavioural applications and sleep regulation in dogs and cats, controlled trials are completely lacking.

Interactions with medication

  • Hormone therapies (thyroid, adrenal cortex, reproduction): possible interaction; use only under veterinary supervision
  • Sedatives and anxiolytics: additive sedative effect possible, combine with caution
  • Corticosteroids: interaction via the HPA axis theoretically possible; clinical relevance in animals unknown

Legal status (Netherlands)

In the Netherlands, melatonin is not registered as a veterinary medicinal product. Use in animals falls under the cascade rule: a veterinary surgeon may prescribe it as a human or magistral formulation when no suitable veterinary medicinal product is available. Use of human supplements without veterinary advice carries a risk of unsuitable excipients (xylitol).
