What is liver disease in dogs?

The liver is a central organ in metabolism: it neutralises toxic substances, produces bile acids and plays a key role in protein and glucose metabolism. 'Liver disease' is not a single diagnosis, but a collective term for various conditions such as hepatitis, liver cirrhosis, toxic liver damage and congenital portosystemic shunts. Causes vary from infections and medicines (such as certain antibiotics or NSAIDs) to autoimmune diseases and cancer.

Because the liver has a large reserve capacity, symptoms are sometimes only visible at a late stage. Early veterinary diagnosis — via blood values, imaging and sometimes biopsy — is essential to establish the cause and start the correct treatment.

Supportive substances: what does the science say?

Silymarin (milk thistle)

Silymarin is the most researched substance for liver support in dogs. It is a flavonoid complex from the milk thistle plant (Silybum marianum) with antioxidant, anti-inflammatory and membrane-stabilising properties [1][2]. Narrative reviews describe biologically plausible mechanisms of action, but independent controlled clinical trials specifically in dogs with liver disease are scarce [1].

SAMe (S-adenosylmethionine)

SAMe is an endogenous molecule involved in methylation reactions and the production of glutathione — an important antioxidant in liver tissue. It is described in the literature as hepatoprotective and is often combined with silymarin in commercial preparations [4].

Combined preparations: silymarin + SAMe

The strongest clinical evidence comes from one randomised controlled study of a commercial preparation combining silymarin with SAMe, in dogs treated with lomustine (CCNU) — a chemotherapeutic agent with known liver toxicity [3]. In that study, a significant protective effect on liver values was measured. Important limitations: small study group, one specific cause of liver damage and commercially initiated research. Extrapolation to liver disease in general is not automatically justified.

Other substances

A broader review from 2020 also discusses phospholipids, vitamin E, curcumin and zinc salts as possible liver-supporting ingredients in complementary feed for animals [4]. For these substances, the evidence in dogs is largely based on in vitro research or extrapolation from human studies — not on veterinary RCTs.

Supplementation is not a replacement

Liver disease in dogs always requires a veterinary diagnosis. Supportive substances can — provided they are assessed and advised by a veterinarian — play a role alongside the primary treatment, not before it or in place of it. The cause of the liver disease determines which treatment is truly necessary; without that knowledge, any supplementation is blind navigation.

Why this verdict?

Grade B is based on one randomised controlled study on silymarin combined with SAMe in chemotherapy-induced liver damage in dogs [3], supplemented by several narrative reviews on silymarin and other liver-supportive molecules [1][2][4]. However, the substantiation is narrow: the only RCT concerns a specific, commercially initiated context with a small study group, and for other causes of liver disease and for individual substances comparable controlled veterinary evidence is lacking.

Interactions with medication

  • Silymarin can affect the action of certain medicinal products via inhibition of liver enzymes (CYP450 system); always consult the veterinary surgeon regarding existing medication [1][2]
  • SAMe may possibly enhance the effect of serotonergic agents; do not combine without veterinary advice [4]

Legal status (Netherlands)

In the Netherlands, complementary animal feeds containing silymarin and SAMe are freely available as dietary supplements for animals. Individual treatment advice — including recommending specific preparations for a diagnosed condition — is reserved for the veterinary surgeon (Wet op de uitoefening van de diergeneeskunde 2018).
