What is colic?
Colic is the umbrella term for acute abdominal pain in the horse. The causes vary widely: from temporary gas accumulation and impaction to life-threatening intestinal strangulations. It is precisely this variation that makes colic dangerous — from the outside, the difference between a mild and a surgical form is not always immediately visible.
Colic is the most common emergency presentation in equine veterinary practice [2]. Survival for medically treatable forms is approximately 87%; for surgically treated colic, this lies between 68 and 80%, strongly dependent on the type of lesion and how quickly surgery takes place [2][3].
Risk factors: what the evidence says
A systematic review [1] identified the following risk factors with the strongest evidence:

Stable management — constant feeding, fixed stabling, gradual transition when changing forage — is thereby the strongest preventive measure that has been shown to work [1].
Additional options: honest per product
Psyllium for sand colic
Psyllium (psyllium husk) is often recommended to remove sand from the large intestine. However, randomised research shows that psyllium as monotherapy is not more effective than a control treatment in horses with confirmed sand accumulation [4]. A combination of psyllium with magnesium sulphate showed better sand clearance in the same study [4]. This means: psyllium alone is insufficient as treatment for sand colic and no substitute for veterinary care.
Probiotics
Probiotics are sometimes used to support the gut flora. The evidence for colic prevention or treatment in the horse is currently insufficient to make a recommendation. Caution is warranted when interpreting product claims.
Herbal remedies and homeopathy
There is no well-designed clinical evidence that herbal preparations or homeopathic remedies effectively treat or prevent colic in the horse. Their use as a replacement for veterinary care is demonstrably dangerous in an emergency condition such as colic.
Timing is everything
In strangulating lesions — such as an intestinal volvulus or a strangulating lipoma — surgical intervention within a few hours is necessary to maximise survival [3]. Delay strongly worsens the prognosis. Early referral to a clinic, before signs of shock occur, is the most important factor for a better outcome [2][3].
In practice, this means: if you doubt whether the situation is improving quickly enough, call the veterinary surgeon immediately and do not wait.
Additional ≠ substitutive
No additional intervention — psyllium, probiotics, herbs — replaces emergency veterinary care for colic. Additional products can at most play a role as part of a maintenance programme approved by the veterinary surgeon (e.g. sand prevention in horses in risk areas), never as a first response to a colic attack.
Why this verdict?
The evidence is based on a systematic review of risk factors [1], a large retrospective cohort study (n=1588) [2] and a multicentre cohort study on surgical outcomes [3]. The quality of evidence is limited because randomised controlled trials in colic are ethically and practically hardly feasible; observational data dominate the field, hence grade B.
Legal status (Netherlands)
Veterinary medicinal products — including analgesics for colic — are available in the Netherlands only on veterinary prescription or to be administered by or under the supervision of a veterinary surgeon.
