What is diet-associated DCM?
Dilated cardiomyopathy (DCM) is a heart muscle disease in which the heart chambers enlarge and pumping function decreases. There are two clearly distinguishable forms:
- Genetic DCM affects breed-specific dogs such as the Dobermann and the Great Dane, regardless of diet.
- Diet-associated DCM (DA-DCM) is a separate syndrome that affects breeds that normally do not have an increased risk — golden retrievers, labradors and mixed breeds — and where diet is suspected to play a role.
What does the evidence say?
Between 2018 and 2022, the FDA recorded more than 1,300 reports of DCM where more than 90% of affected dogs ate grain-free, legume-rich food [4]. The FDA concluded its investigation in December 2022 without a proven causal link, but initiated follow-up research.
A prospective study in golden retrievers showed that dogs on a non-traditional (grain-free/legume-rich) diet had a 9.8 times higher chance of taurine depletion and a 15 times higher chance of echocardiographic abnormalities consistent with DCM, compared with dogs on grain-inclusive food [1].
A controlled 28-day feeding study found that wrinkled peas caused subclinical changes in heart function — reduced stroke volume and enlarged left ventricle — without a change in plasma taurine [2]. This suggests that the mechanism extends beyond just a taurine deficiency; which substance or process exactly is responsible remains unclear.
Important nuance: other studies in labradors after 26 weeks of grain-free food found no deteriorated taurine status. The evidence is therefore not entirely consistent [3].
Recovery after diet change
A clinical characteristic that distinguishes DA-DCM from genetic DCM is that most patients improve significantly after switching to grain-inclusive food, in combination with taurine supplementation. Echocardiographic recovery occurs within months. In genetic DCM (Dobermann, Great Dane), heart function does not respond to this [3].
This makes diet change and taurine supplementation the core of the current approach to DA-DCM — but both are always supervised and monitored by a (veterinary) cardiologist.
Precautionary principle
Because causality has not yet been proven but the signals are consistent, most veterinary cardiologists recommend giving grain-inclusive food to non-predisposed breeds — particularly golden retrievers — as long as the question remains unresolved. This is a precautionary measure, not proven prevention.
Adjunctive ≠ replacement
DCM is a serious, life-threatening condition. Diet change and taurine supplementation are adjunctive to veterinary treatment, never a replacement for it. Cardiac medication (diuretics, ACE inhibitors, antiarrhythmics) remains necessary in advanced cases. Never delay a veterinary consultation if your dog shows symptoms.

Why this verdict?
The evidence is based on a prospective observational study (Ontiveros et al., 2020), a controlled 28-day feeding study (Quilliam et al., 2023) and more than 1,300 FDA reports — no prospective RCTs, meaning causality is not yet proven and not all findings are consistent.
Interactions with medication
- Taurine supplementation may affect the requirement for cardiac medication — always coordinate with the veterinary surgeon
- Diuretics (in heart failure) combined with a diet change require re-evaluation of electrolyte balance
Legal status (Netherlands)
The FDA investigation into the relationship between grain-free diets and DCM was concluded in 2022 without proven causality; no sales bans or recall actions have been issued for specific feed brands.
