What is Co-enzyme Q10?

Co-enzyme Q10 (CoQ10, also known as ubiquinone or in the reduced form ubiquinol) is a fat-soluble compound that the body produces itself. It plays a key role in the mitochondria: it transports electrons in the respiratory chain and is therefore essential for the production of cellular energy (ATP). In addition, CoQ10 acts as an antioxidant and protects cells against oxidative damage.

Due to its continuously high energy demand, heart muscle tissue has a particularly strong dependence on CoQ10. In dogs with heart disease, it has been investigated whether deficiencies play a role and whether supplementation may be useful.

Evidence in dogs

Heart disease (MMVD)

Myxomatous mitral valve degeneration (MMVD) is the most common heart disease in dogs, and the Cavalier King Charles Spaniel is genetically extra vulnerable.

A case-control study [3] showed that CoQ10 levels in heart tissue are significantly reduced in CKCS dogs that have developed heart failure due to MMVD. This provides a biological rationale for supplementation, but does not in itself prove that supplementation also helps.

Two RCTs subsequently investigated what supplementation does:

  • A crossover RCT [1] in 19 CKCS dogs with MMVD showed that CoQ10 supplementation significantly increased plasma levels (from an average of 0.92 to 3.51 µg/ml). After three weeks, however, there were no significant improvements in echocardiographic parameters (heart size, pump function).
  • A second double-blind randomised study [2] confirmed that a higher daily dose is the most effective way to achieve an approximately threefold plasma increase in dogs with MMVD and heart failure.

Conclusion: CoQ10 demonstrably increases plasma levels in dogs with MMVD, but the evidence that this leads to measurable improvement in heart function or survival is currently insufficient. The studies are small and focused on pharmacokinetics, not on clinical endpoints.

Absorption and form

Ubiquinol (the reduced form) has better bioavailability than ubiquinone. Individual variation in absorption is large. Administering CoQ10 with a fat-rich meal significantly increases uptake.

Application and limitations

CoQ10 is used by some veterinary surgeons as additional support in dogs with heart disease, alongside proven veterinary treatments. It never replaces registered heart medication such as pimobendan, diuretics or ACE inhibitors — these do have strong clinical evidence for improvement of survival and quality of life.

In healthy dogs or for other indications (energy, ageing, cognition), evidence from controlled studies is almost entirely lacking.

Safety

CoQ10 is generally well tolerated in dogs. Adverse effects are rare and mild (sometimes gastrointestinal complaints). Always administer the dosage based on advice from a veterinary surgeon, as the correct amount depends on weight, disease stage and any concomitant medication.

Interaction with blood thinners: There are indications that CoQ10 may reduce the effect of warfarin (lowers INR). In dogs receiving blood thinners, consultation with the veterinary surgeon is mandatory before use.

Why this verdict?

Two randomised controlled studies in Cavalier King Charles Spaniels confirmed that CoQ10 supplementation significantly increases plasma levels; a case-control study showed depletion in heart tissue in heart failure. The studies are small (n=19 in the largest RCT) and primarily measure pharmacokinetics, not survival or clinical outcome — hence grade B: reasonable evidence for biological plausibility, insufficient for a definitive efficacy claim.

Interactions with medication

  • Warfarin and other coumarin-based blood thinners: CoQ10 can lower INR and thereby reduce the efficacy of anticoagulants — monitoring required
  • Cardiac medication in general: CoQ10 is solely adjunctive; it does not replace pimobendan, furosemide or ACE inhibitors and must never do so

Legal status (Netherlands)

In the Netherlands and Belgium, CoQ10 is available as a dietary supplement for pets without a prescription. It is not a registered veterinary medicinal product. Veterinary supervision is recommended, particularly for cardiac patients.
