What is FIC?
Feline Idiopathic Cystitis (FIC) is a recurring condition in which a cat shows symptoms of a bladder infection — frequent visits to the litter tray, painful urination, blood in the urine — without a bacterial infection being demonstrable [3]. "Idiopathic" literally means: without known cause. Yet one factor is always prominently present: stress.
FIC resolves on its own in most cats within 5–7 days. However, without addressing the underlying stress factors, relapse is almost certain [2].
Why stress?
Cats are sensitive to disturbances in their environment: conflicts with other pets, changes in routine, a monotonous living space or the feeling of having little control over their environment demonstrably increase the risk of FIC [3]. The nervous system and the bladder are more closely connected in cats than in most other animals; chronic stress demonstrably leads to changes in the bladder wall.
What works — and what does not
Environmental enrichment (MEMO) — strongly recommended
Multimodal environmental modification (MEMO) is the best proven approach. In a prospective study (Buffington et al., n=46), the relapse rate decreased from weekly to never in 72% of cats after an average of 300 days [1]. The core consists of the five pillars of the cat environment:
- Safe space to retreat to
- Sufficient resources per cat (litter trays, feeding points, resting places)
- Play opportunities and active stimuli
- Choice in human contact (do not force)
- Odour and sensory enrichment
Increased water intake — recommended
More fluid dilutes the urine and reduces the severity and duration of episodes [2]. In practice:
- Switch to wet food (or add wet food to the ration)
- Drinking fountain or multiple drinking places in quiet locations
- Place water bowls far from the litter tray
Antibiotics — not useful in FIC
Because there is NO bacterial infection present, antibiotics have no added value in FIC [3]. They are therefore not recommended as standard treatment.
GAG supplements (glycosaminoglycans) — insufficient evidence
Supplements intended to strengthen the protective layer of the bladder are popular, but there is no convincing evidence that they prevent or reduce FIC [2]. They are not harmful, but expectations must be realistic.
Painkillers and anti-inflammatories — not routinely
Anti-inflammatory agents are not routinely recommended as standard treatment [2]. In cases of severe pain, a veterinary surgeon may prescribe something for a short period; this is an individual decision.
Amitriptyline — only in severe refractory cases
This antidepressant is only considered if all other measures fail and the cat has chronic severe complaints — not as a first step [2].
Supplemental ≠ replacement
Environmental adjustments and dietary adjustments are supplemental to veterinary care, not a replacement for it. In the case of a first episode, severe complaints or male cats, a veterinary consultation is always necessary to rule out urethral obstruction or other conditions.
Why this verdict?
The grade B assessment is based on a prospective clinical study (n=46) on multimodal environmental enrichment [1] and a systematic review from 2025 [2]. The studies are methodologically limited in size and control groups; large-scale randomised controlled trials are still lacking.
